To provide evaluative, consultative and treatment interventions to patients referred by a licensed physician.
To maintain effective communication with center/agency staff, patients, families, and management.
Essential Skill/Knowledge Functions: Assessment, evaluation and treatment of referred patients Appropriate and timely development of Plan of Care of patients served Appropriate and timely documentation of progress towards goals as outlined in United Rehab Policy and Procedure manual Appropriate and timely communication of patient status to members of the Interdisciplinary treatment team Appropriate referrals to members of the Interdisciplinary treatment team Supervision of all clinical activities of the Licensed Physical Therapy Assistant according to State Practice Acts.
Provision of formal and informal feedback to Licensed Physical Therapy Assistant according to State Practice Acts.
Supervision of Rehab Aide Orders and/or assists in obtaining patient equipment and/or supplies to attain patient-related therapeutic goals.
May modify equipment, appliances and/or splints to meet patient-related therapeutic goals Participates in conducting in-services to rehab and/or center staff regarding Physical Therapy interventions.
Submits billing, payroll, and expense documentation in a correct and timely manner.
Essential Administrative Functions: Participate in center/agency surveys (Licensure / JCAHO) and any subsequently required reports.
Attend and participate in continuing educational programs to keep abreast of changes in your field as well as to maintain current license/certification, as required.
Attend and participate in mandatory in-services.
Honor patients/residents' rights to fair and equitable treatment, self-determination, individuality, privacy, property and civil rights, including the right to wage complaints.
Comply with corporate compliance program.
Report job-related functions/tasks that involve occupational hazards including exposure to blood and body fluids and others as necessary.
Follow established safety regulations, to include fire protection & prevention, smoking regulations, infection control, etc.
Follow established safety procedures when performing tasks and/or working with equipment.
Perform other related duties as necessary and as directed by supervisor.
Licensure/Certification/Education Requirements: Graduate of CAPTE accredited school.
If foreign trained, the program must be deemed substantially equivalent to a Physical Therapist entry level education in the U.
by a credentials evaluation organization approved by the APTA.
Valid license or eligibility in the State of practice Other Training, Skills, and Experience Requirements: Previous experience in long term care preferable.
Must be able to speak, read, and write the English language The ideal candidate will have at least one-year work experience treating the adult population.

The fraudster will send a check to the victim who has accepted a job. The check can be for multiple reasons such as signing bonus, supplies, etc. The victim will be instructed to deposit the check and use the money for any of these reasons and then instructed to send the remaining funds to the fraudster. The check will bounce and the victim is left responsible.
